Colorado Nursing Home Admissions

Once a home has been agreed upon, a basic evaluation is made to determine if the person's needs can be satisfied, availability is checked, and form of payment agreed upon most of the hard work will be done. Then the potential resident or guardian fills out an application for admission to the home. A copy of guardianship, power of attorney, trustee appointment, living will, or health care directives need to be provided to the residence before being admitted.

Admission Criteria

Each nursing home is required by law to provide a personal care plan for each resident. Family members and those close to the resident are encouraged to take part in the care plan. Residents of all creeds, religions, sexs and races are welcomed to our homes. A physical examination shall be done at the time of admission and at least annually thereafter for each resident admitted.

Arrangements for admission are to be made by the family member or legal guardian. There must be a legally responsible individual to sign the authorizations and/ or agreements and to make decisions if the resident is unable. A review of the Resident's Bill of Rights has to be made by the resident or the legal guardian with a statement signed that the rights are understood / acknowledged. A resident is responsible for his or her own external affairs unless a full guardianship is in effect. Payment for care is expected 30 days prior to service rendered unless otherwise agreed. Medications brought into the nursing home must be approved by a doctor before staff can administer the medication. Where you purchase medications is your choice.
